With this application you must submit the following items:
- Letters of recommendation - indicating satisfaction with your product or service. Letters must be current and written specifically to MERSC. The name and phone number of the person writing the letter must be included on the letter.
- Liability Insurance - Provide verification of liability insurance.
- Federal Form W9
- Membership and Application fee checks / credit card info
